Learn how to manage your audit process using the csv templates. Depending on the type of feedback provided, there are three types of CSV templates:

• HCP Entity Adhoc is used to change a provider’s name, demographic information, or practitioner type.
• HCP Source Adhoc is used to add or change an NPI, State License or CSR record.
• The HCP Link Adhoc is used to verify Date of Birth, Date of Death Status, DEA, State License, CSR, NPI or Sanctions.
